MVP Equipment Rental Release Download/Upload Page
The MVP's insurance carriers now require a release form be signed by all
individuals renting MVP equipment yearly. (ie, Not for every rental.)
The rental code will check and not allow you to rent anything unless
there is a signed form on file for the current year.
Instructions:
1. Download and print the release form:
MVP Equipment Rental Waiver and Release of Liability (with Safety Requirements)
2. Fill it out and sign it.
3. Scan it or take a photo of it.
4. Upload below.
